Vikas Bharat 2047 – In a significant advancement for neurological rehabilitation in India, researchers from the Indian Institute of Technology (IIT) Delhi, All India Institute of Medical Sciences (AIIMS) New Delhi, and the Institute of Human Behaviour and Allied Sciences (IHBAS) have developed the country's first Hindi adaptation of Melodic Intonation Therapy (MIT), a music-based speech rehabilitation technique designed to help stroke survivors recover their ability to communicate.

India's First Hindi Music-Based Speech Therapy – A Major Challenge After Stroke

The innovative therapy, tailored specifically for Hindi-speaking patients, has shown encouraging results in a pilot study published in the internationally reputed journal Aphasiology. Researchers believe the development could transform speech rehabilitation for millions of stroke survivors across India who experience aphasia, a condition that affects a person's ability to speak after brain injury.

Stroke remains one of the leading causes of disability in India, affecting thousands of people every year. While many survivors recover physically over time, a considerable number continue to struggle with communication due to post-stroke aphasia. Among the different forms of the condition, non-fluent aphasia is particularly challenging because patients often understand conversations but find it extremely difficult to produce words or complete sentences.

Individuals suffering from this condition frequently retain the remarkable ability to sing familiar songs despite losing the capacity for normal speech. This preserved musical ability has inspired researchers around the world to develop therapeutic techniques that combine rhythm, melody and speech to stimulate language recovery.

Melodic Intonation Therapy has been successfully used for decades in English and several Western languages. However, until now, there had been no scientifically validated Hindi version that considered the unique rhythm, pronunciation and natural cadence of one of India's most widely spoken languages.

Why Hindi Adaptation Matters

Hindi is the primary language for hundreds of millions of Indians. Despite this, most speech rehabilitation programmes available in the country have traditionally relied on English-based materials that do not accurately match the linguistic characteristics of Hindi.

Experts say that simply translating words is insufficient because every language has its own patterns of stress, melody and rhythm. Since Melodic Intonation Therapy relies heavily on musical elements to stimulate speech recovery, adapting the therapy to suit Hindi required a complete redesign rather than a direct translation.

The research team therefore developed an entirely new Hindi version of MIT that reflects the language's natural pronunciation, intonation and rhythmic flow. According to the researchers, this culturally and linguistically appropriate approach has the potential to improve patient engagement and treatment outcomes.

How the Melodic Intonation Therapy Was Developed

The project involved a multidisciplinary team comprising engineers, neurologists, speech therapists, ENT specialists and rehabilitation experts. Their first objective was to redesign the therapy exercises so they aligned with the linguistic structure of Hindi.

Instead of merely replacing English vocabulary with Hindi words, researchers carefully modified melodies, rhythmic patterns and speech exercises to correspond with the natural characteristics of spoken Hindi. This ensured that patients practised speech patterns they would commonly use in their daily lives.

Following the development of the Hindi adaptation, six Hindi-speaking stroke survivors diagnosed with non-fluent aphasia participated in the pilot feasibility study.

Promising Results from the Pilot Study

Each participant completed a structured rehabilitation programme consisting of 40 therapy sessions lasting between 45 and 60 minutes. The sessions were delivered by a trained speech therapist using the newly developed Hindi Melodic Intonation Therapy framework.

Researchers reported that all six participants successfully completed the programme and demonstrated clinically meaningful improvements in speech and communication abilities.

The improvements were not limited to laboratory assessments. Participants showed noticeable gains in everyday communication, including the ability to produce longer sentences, speak more naturally and interact with greater confidence in daily situations.

Researchers also observed improvements in the participants' quality of life, suggesting that enhanced communication positively influenced their emotional well-being and social interactions. Importantly, no adverse effects were reported during the study.

Researchers Highlight Encouraging Outcomes

According to Prof. Rahul Garg, one of the principal investigators from IIT Delhi, the results indicate that patients experienced meaningful improvements in their communication abilities.

He noted that participants were able to construct longer sentences, speak with greater fluency and reported improvements in their overall quality of life after completing the therapy programme. The absence of any treatment-related complications further supports the feasibility of the Hindi adaptation.

However, the researchers emphasised that the findings should be interpreted cautiously because of the limited sample size.

Prof. N. M. Anoop Krishnan explained that although the outcomes are encouraging, the study involved only six patients and therefore represents an important preliminary step rather than definitive clinical evidence. Larger clinical trials involving more participants will be necessary to establish the therapy's effectiveness across diverse patient populations.

Expanding Access Through Digital Innovation

Beyond the successful pilot study, the research team is also working on digital solutions that could make the therapy widely accessible across India. Access to specialised speech therapists remains limited in many regions, particularly in smaller towns and rural areas.

Digital platforms could eventually enable patients to receive guided therapy remotely while remaining under the supervision of trained healthcare professionals. Such technology-driven rehabilitation could significantly improve access to speech therapy for stroke survivors who currently face geographical or financial barriers.

The researchers believe that combining evidence-based rehabilitation with digital healthcare technologies could help bridge the gap in neurological rehabilitation services throughout the country.

Interdisciplinary Collaboration Drives Innovation

The study reflects the growing importance of interdisciplinary collaboration in healthcare research. The project was jointly led by Prof. N. M. Anoop Krishnan and Prof. Rahul Garg from IIT Delhi, along with Prof. Deepti Vibha from AIIMS New Delhi.

Mr. Dushyant Soni, a PhD scholar at IIT Delhi's National Resource Centre for Value Education in Engineering (NRCVEE), and Ms. Rupanshi Ahuja served as joint first authors of the study.

The wider research team also included Ms. Subhasmita Tripathy, Ms. Shivani Aggarwal, Dr. Kapil Sikka from the Department of Otorhinolaryngology and Head and Neck Surgery at AIIMS, and Dr. Suman Kushwaha from the Department of Neurology at IHBAS.

The collaboration combined expertise from engineering, neurology, speech-language pathology, rehabilitation medicine and otorhinolaryngology, demonstrating how multidisciplinary research can address complex healthcare challenges.

Potential Impact Across India

The development of India's first Hindi version of Melodic Intonation Therapy marks an important milestone in speech rehabilitation. With Hindi being one of the country's most widely spoken languages, the therapy could eventually benefit a large population of stroke survivors who previously lacked access to language-specific rehabilitation techniques.

While larger clinical trials are needed before widespread implementation, the pilot study provides encouraging evidence that language-specific adaptations of internationally recognised therapies can improve treatment outcomes.

The findings also highlight the importance of developing healthcare interventions that are tailored to India's linguistic and cultural diversity. Researchers hope that future work will extend similar adaptations to other Indian languages, ensuring that stroke survivors receive rehabilitation in the language they know best.
